Go to https://trailhead.salesforce.com and sign in with your Trailblazer account.

Click your profile avatar in the top right corner.
From the dropdown, select Profile to access your public Trailblazer page.

Once verified, Trailhead will sync your certifications to your Trailblazer profile automatically.
